All 2024-2025 opt-out refund requests received via the Opt-out Refund Request Form have been processed.

If you placed your request via email and have not received your refund:

1) Complete the form, and;
2) Email us at [email protected]

We do not receive automatic notification of your requests; this program has now ended and our intent is to close out all refunds no later than December 31, 2025.
